    What Do I Need to Bring to My Appointments?

    Please bring a photo ID and proof of Insurance to every visit. Any medical records that you feel are relevant to your allergy visit may also be helpful.

    How Early Do I Need to Arrive For My Appointment?

    First time patients please arrive 30 minute prior to your scheduled appointment time to allow sufficient time to fill out new patient paperwork. Returning patients or follow up visits please arrive 15 minute prior to your scheduled appointment time.
